Care Manager II — Lead Patient Care & Transitions (12-hr)

Englewood Health in Englewood, NJ is seeking a Care Manager to oversee patient care from admission through post-discharge, coordinating with nurses, therapists and multidisciplinary teams to ensure safe, efficient transitions and high-quality outcomes. Requirements include a BSN (MSN preferred), NJ RN license and BLS; 2 years Med/Surg experience (Ortho/Neuro preferred).
