Physical Therapist

In this role, you will deliver one-on-one, evidence-based physical therapy to former energy workers and their families, helping clients maximize mobility, reduce pain, and improve their overall quality of life in the comfort of their homes. You will conduct comprehensive evaluations, develop and implement individualized treatment plans, and regularly reassess progress, adjusting interventions as needed. Working closely with nurses, physicians, case managers, and other members of the interdisciplinary care team, you will collaborate to ensure coordinated, patient-centered care that supports both short- and long-term functional goals. As a Physical Therapist with Professional Case Management, you will be responsible for teaching patients and caregivers therapeutic exercises, safe transfer techniques, and the proper use of assistive devices and durable medical equipment. You will provide thorough education on fall prevention, energy conservation, and home safety, tailoring your approach to each client’s medical condition, home environment, and personal goals. Accurate and timely documentation in electronic health records is essential, including visit notes, progress reports, and communication with physicians and payers. You will adhere to all regulatory, ethical, and company standards of practice while managing your schedule efficiently to meet visit expectations and response times.
