Bilingual Geriatric Care Coordinator (English/Spanish)

Human Service Forum

Human Service Forum is seeking a full-time case manager for the Primary Care Team in Springfield, Massachusetts. This hybrid position requires fluency in Spanish and English and involves coordinating services for members of the Senior Care Option plan to improve their health and independence. Key responsibilities include conducting assessments and collaborating with community resources.
