Hospice RN Case Manager: Lead Compassionate Home Care

Hospice Plus

Hospice Plus is seeking a dedicated RN Case Manager to lead patient-centered hospice care in home settings across Texas. You will perform assessments, develop plans of care, coordinate with physicians, nurses, social workers and chaplains to ensure compassionate, evidence-based support. This role emphasizes education for families, on-call participation, and meticulous documentation while adhering to regulatory standards. A current RN license, CPR certification, and driving access are required. #J-18808-Ljbffr Hospice Plus
