Job Summary With a commitment to patient- and family-centered care, the professional nurse uses the nursing process to honor the uniqueness of each individual and address the physical, psychological, emotional, and social needs of the diverse patients served by Jefferson Health Ambulatory Clinics. As an integral member of the ambulatory care clinical team, the nurse collaborates with providers and support staff, delivers clinical nursing interventions, and communicates respectfully with patients to address individualized needs and priorities consistent with cultural beliefs and values. Must maintain a current nursing license in the state of practice. Work Shift: Workday Day. Job Duties Integrates evidence-based practice, clinical expertise, patients, and their family/designated care partners' preferences and cultural beliefs for delivery of optimal healthcare. Collaborates and communicates with clinic care providers regarding patient care and management; supports efficient and timely communication between primary care providers and specialists. Supports implementation of clinical care pathways for common care conditions and participates actively in quality improvement opportunities. Utilizes evidence-based ambulatory care nursing interventions during clinical visits, telephone encounters, and electronic communication. Provides patient, family, and care partner education for preventive care, chronic disease management, medication management, and management of common illnesses treated in the primary care setting. Documents all patient and family interactions and clinical care in the electronic medical record (EMR) and provides nursing care according to organizational standards and applicable regulations. Performs care coordination for patients (e.g., abnormal results, triage, RN verification of vaccinations and other injections, management of in-baskets, prior authorizations and forms). Oversees clinical care essential to the assessment, promotion, maintenance, and restoration of patients’ health and wellness. Minimizes risk of harm to patients and care partners by following established nursing practices and protocols. Uses information technology to communicate, manage knowledge, and mitigate errors. Uses data to monitor outcomes of care processes and supports ethical decision making; applies improvement methods to design and test changes to continuously improve the quality and safety of patient care. Functions effectively within ambulatory care inter-professional teams, fostering open communication, mutual respect, and shared decision-making to achieve quality patient care. Recognizes patients and care partners in providing safe, compassionate, and coordinated care based on respect for their preferences, priorities, needs, and cultural beliefs and values. Creates an environment of belonging and inclusion and demonstrates skills in problem solving and conflict resolution. Demonstrates a commitment to learning and seeks opportunities for continued professional growth and development. Actively participates in team huddles/assemblies to provide announcements and clinical operations updates. Demonstrates willingness to give and receive feedback and integrates appropriate changes into practice. Minimum Qualifications Specialized Diploma Graduate of an accredited school of nursing required. Patient-and-family-centered practice knowledge of how to convey respect and collaborate with the patient and the designated care partner in care and care planning. Cultural awareness knowledge of people with diverse backgrounds and the ability to convey respect and connect effectively. Effective communication understanding of effective communication concepts, tools, and techniques; ability to transmit, receive, and accurately interpret ideas, information, and needs through the application of appropriate communication behaviors. Uses the language of partnership with patients, engaging them in their health and healthcare. Appropriately and respectfully delegates to other team members. Professionalism exhibiting honesty, confidentiality, and integrity in all activities and communications. Attention to detail and effective time management. Maintains confidentiality and adheres to HIPAA. Demonstrates ethical behavior as defined in the ANA Code of Conduct. Demonstrates willingness to precept nursing students. Patient assessment and care knowledge of nursing and medical terminology; ability to assess ailments and injuries of patients to triage appropriately. Patient and designated care partner education knowledge of child and adult education techniques and health literacy barriers. Ability to elicit learning priorities and needs and educate patients and their designated care partners on the patient's condition, the treatment and medications suggested and given, and the next steps in recovery. Medication administration and prescription knowledge and application of pharmacology as it relates to primary care practice including medications used to treat patients and immunizations for preventive care. Knowledge of pharmacology resources and references for professional and patient use. Assessment and diagnosis knowledge of various tools and techniques for patient assessment and nursing diagnosis or clinical judgment; ability to explore and analyze reasons behind symptoms and triage patients effectively. Health information documentation knowledge of the relevance of health information and appropriate documentation methods; ability to record medical histories, symptoms, vital signs, medications, diagnoses, treatment plans, procedures, tests, and medical results in written or electronic form. Complete EMR training and remain proficient and knowledgeable with EMR updates. Utilize and remain current with the tasking function of the EMR. BLS – Basic Life Support (AHA). RN – Licensed Registered Nurse, PA – State of Pennsylvania. Preferred Qualifications Bachelor’s Degree (BSN) or completion of BSN or an appropriate advanced degree within three years of hire strongly preferred. One year experience as a nurse in an ambulatory setting. Additional Qualifications Encouraged to obtain ambulatory care nursing certification. Expected to participate in or support quality, evidence-based practice and research of nursing governance initiatives. Physical Demands Lift and carry 40‑50 lbs. Examples: push/pull patients on bed, stretcher (requires 29‑lb push force), lateral transfers up to 50 lbs. of the patient’s weight. Frequent to continuous standing/walking. Patient transporters may walk 8‑10 miles per shift. Patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting, and performing CPR.
