Job Details Conduct physical and psychosocial assessments for hospice and palliative care patients in community and facility settings. Develop, implement, and adjust personalized nursing care plans to meet patient needs. Electronically document patient visits and maintain accurate medical records. Collaborate with interdisciplinary teams to ensure comprehensive, patient-centered care. Provide guidance, support, and education to patients and their families. Participate in on-call coverage, including one weekend per month and 2-3 backup on-call shifts per month. Offer compassionate end-of-life care that emphasizes dignity and comfort. Skills You Need Associate's degree or diploma from an accredited nursing program; BSN preferred. Active NYS Registered Nurse (RN) license. Minimum of 2 years of experience in med/surg, community health, or oncology nursing preferred. Valid NYS driver's license and willingness to travel for patient visits.
