The Hamptons Center for Rehabilitation and Nursing - Social Worker Facility: The Hamptons Center for Rehabilitation & Nursing Position: Social Worker Location: Southampton, NY Shift: Full Time Salary: $65,000–$75,000 per year Join Our Team! The Hamptons Center for Rehabilitation & Nursing is seeking a compassionate and dedicated Social Worker to support the psychosocial needs of our residents and their families. The Social Worker will collaborate with residents, families, and the interdisciplinary team to develop care plans, coordinate services, and support successful transitions and discharge planning. Responsibilities Assess residents’ social, emotional, and psychosocial needs and develop appropriate care plans. Maintain accurate and timely social work documentation and resident records. Provide support and intervention to residents and families experiencing personal or environmental challenges. Coordinate and participate in family meetings and interdisciplinary care planning. Assist with discharge planning and coordinate services with outside agencies. Serve as a liaison between residents, families, staff, and community resources. Collect pertinent social information upon admission. Monitor and document social services provided. Promote residents’ overall well-being and quality of life. Perform other duties as assigned. Requirements Bachelor’s degree in Social Work or a related human services field, including Sociology, Psychology, Rehabilitation Counseling, or Special Education. Minimum 1 year of experience in social work, preferably in a healthcare or long-term care setting. Knowledge of family assessment and intervention preferred. Strong written, verbal, and interpersonal communication skills. Knowledge of governmental regulations related to social services. Ability to work independently, problem-solve, and make sound decisions. Ability to work effectively with residents, families, staff, visitors, and outside agencies.
