Specialty Medicine RN Care ManagerHelp make a meaningful impact on the health and well-being of Veterans as a Specialty Medicine RN Care Manager at the Sioux Falls VA Health Care System. In this collaborative role, you will help Veterans and their families understand their care options, navigate transitions between health care settings, and access the services and resources needed to achieve the best possible outcomes. Working closely with providers and interdisciplinary teams, you will bring together clinical assessment, patient education, advocacy, and care coordination to promote continuity, strengthen self-management, and support personalized care for Veterans with a wide range of acute and chronic health conditions.Duties include, but are not limited to:Coordinate comprehensive nursing care for Veterans with acute and chronic health conditions across multiple care settings and caregivers.Assess Veteran needs within the context of family, community, living conditions, cultural background, and available support systems to develop individualized plans of care.Facilitate safe transitions after hospitalization by evaluating nursing care needs, identifying appropriate community resources, and supporting post-hospital planning.Conduct post-discharge follow-up to reinforce medication instructions, discharge plans, follow-up appointments, and the appropriate level of ongoing care.Educate Veterans and families about healthy practices, treatment options, and self-management strategies that support informed health care decisions.Triage Veteran concerns using established clinical protocols, assess changes in condition, and coordinate appropriate follow-up care.Advocate for Veterans by clarifying care options, supporting decision-making, addressing barriers to care, and collaborating with the interdisciplinary team to resolve complex needs.Evaluate care outcomes and identify opportunities to improve the quality, efficiency, continuity, and delivery of Veteran-centered care.VA offers a comprehensive total rewards package: VA Nurse Total RewardsPay: Competitive salary, regular salary increases, potential for performance awardsPaid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VAInsurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)Licensure: 1 full and unrestricted license from any US State or territoryWork Schedule: Monday - Friday 7:30am - 4:00pmTelework: Not AvailableVirtual: This is not a virtual position.Relocation/Recruitment Incentives: Not AuthorizedPermanent Change of Station (PCS): Not AuthorizedTravel Requirements: You may be expected to travel 25% or less of the time for this position to various care locations within the service area, including Veteran homes as part of Home Health care delivery. You must possess and maintain a current, valid, unrestricted state-issued driver's license. You must provide a safe, clean driving record from the Department of Motor Vehicles (DMV) during onboarding.
