Key Responsibilities

Evaluate and treat patients in a skilled nursing facility setting to restore or maintain functional independence
Develop and implement individualized occupational therapy treatment plans based on patient needs and goals
Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory standards
Collaborate with interdisciplinary care teams to ensure comprehensive patient care
Educate patients and caregivers on therapeutic techniques and adaptive strategies to support daily living activities

Required Qualifications

Valid Occupational Therapist (OT) license in the state of Oregon
Graduate of an accredited Occupational Therapy program
Prior experience working in a skilled nursing or long-term care environment
Strong documentation and communication skills

Preferred Experience / Skills

Experience with geriatric patient populations
Familiarity with MDS documentation and Medicare compliance in SNF settings
Experience with ADL retraining and adaptive equipment recommendations
