Physical Therapist

Provide one-on-one physical therapy to older adults in private residences and senior living communities Conduct evaluations, develop personalized care plans, and deliver evidence-based treatments Support patient wellness, mobility, independence, and aging in place Document care using EMR systems in compliance with regulations Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and Fitness Specialists Educate patients, caregivers, and staff on safety and functional aging strategies Utilize telehealth/virtual care to enhance access and flexibility Role: Physical Therapist Location: O‘Fallon, IL Responsibilities: Provide one-on-one physical therapy to older adults in private residences and senior living communities Conduct evaluations, develop personalized care plans, and deliver evidence-based treatments Support patient wellness, mobility, independence, and aging in place Document care using EMR systems in compliance with regulations Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and Fitness Specialists Educate patients, caregivers, and staff on safety and functional aging strategies Utilize telehealth/virtual care to enhance access and flexibility Qualifications & Skills: Valid Physical Therapy license or eligibility in the state Degree from an accredited PT program Proficiency with computers and EMR systems Current CPR certification Physical Requirements: Ability to exert medium physical effort, including lifting, repositioning patients, and assisting with transfers Capability to perform CPR, stand, walk, climb stairs, and drive between patient locations Wearing PPE when necessary Key Attributes: Autonomy, flexibility, strong patient relationships, and commitment to improving community health.
