Empath Home Health , a part of Empath Health is seeking a compassionate full‑time Home Health PT Physical Therapist to work with us to provide individualized care in Hendry County (Clewiston, LaBelle, Lehigh Acres, and surrounding areas). Empath Home Health provides skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support that assists with daily needs as part of a seamless continuum of care. Locations: Home health visits in Hendry County (including Clewiston, LaBelle, Lehigh Acres, Moore Haven, Buckingham , and surrounding areas). Schedule: Full‑Time; M‑F 40 hours with required weekend/holiday rotations as needed. What You’ll Do Provide physical therapy services to patients as prescribed by a physician in compliance with agency policies and procedures and federal and state regulations. Perform initial and ongoing assessments, including OASIS assessments at appropriate time points, to determine level of functioning and patients’ ability to perform activities of daily living. Develop and revise plans of care, in consultation with the physician and other care team members, to enable patients to achieve realistic goals of mobility. Report to the physician and designated agency representative about patients’ reactions to treatment and any changes in the patients’ condition. Minimum of 1 year experience as a licensed Physical Therapist (PT). Home health care experience strongly preferred. Strong assessment, critical thinking, and patient education skills.
