Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: - Minimum 2 years of Acute Care Case Management or Discharge Planning experience required. - Hospital experience is mandatory; behavioral health experience does not apply. - Traveler must be seasoned and have experience with acute care settings. License: - Current state nursing license required. - Pending licenses accepted, but must obtain full licensure before starting. Certifications: - Relevant nursing and case management certifications preferred. Must-Have: - Experience with assessing patients, care coordination, and discharge planning. - Knowledge of community resources, patient advocacy, and utilization review. - Competence in arranging durable medical equipment and transport services. Description: This position involves managing a caseload of patients and coordinating their care. The Case Manager will assess patient needs and develop comprehensive care plans. Collaboration with interdisciplinary teams is essential to ensure optimal patient outcomes. Participation in family meetings and treatment plan development is required. Experience with health plans is beneficial, as is familiarity with complex patient scenarios. The Case Manager will work with a variety of patient age groups including neonates, adolescents, adults, and geriatrics. Ability to organize and communicate effectively in a fast-paced environment is crucial.
