Social Worker — Landmark Hospital of Joplin, MO

About the Social Worker Role

As our Social Worker, you'll work on site in Joplin and partner with physicians, nurses, and case managers to plan safe, timely discharges for medically complex patients. You'll be a key advocate for patients and families — helping them understand their options, connect with the right resources, and move confidently into the next stage of care.

What You'll Do

• Support patients and families through discharge planning, financial concerns, and the emotional impact of acute illness
• Provide supportive counseling as patients adjust to illness and address broader biopsychosocial needs
• Coordinate with external agencies to ensure a smooth, well-informed transition out of the hospital

What is Required to be a Social Worker @ Landmark Hospitals

• Bachelor's degree in Social Work required; Master's degree preferred
• Active, unrestricted Missouri LMSW or LCSW preferred
• 5+ years of hospital-based social work experience
• Strong working knowledge of community resources, referral agencies, and healthcare systems
• Sharp analytical and critical-thinking skills, with the ability to manage multiple complex cases at once
• A collaborative, empathetic approach and excellent communication skills across a multidisciplinary team
