Physical Therapist - Travel Contract

Minimum Requirements Associate, Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA. Active Kentucky Physical Therapy license required to start the assignment. Home health experience required. Experience with OASIS assessments and documentation using HCHB required. Experience performing admissions, recertifications, 30-day assessments, and discharges in a home health environment. Experience supervising PTAs or coordinating care with PT assistants required. BLS certification from AHA or ARC may be required. Minimum education requirement: Associate degree (client requirement). Preferred: prior travel or remote caseload management experience. Preferred: familiarity with mileage reimbursement processes between visits. Assignment Details Facility type: Home Health. Shift: Days. Schedule: Monday through Friday, typical daytime hours (client indicates Mon–Fri am to 4:30 pm). Weekly guaranteed hours: 32 hours per week. Productivity expectation: 30 points. Coverage area: Logan and Todd counties. Mileage reimbursed between first and last patient visits. On-call/weekend requirement: No. Dress code: royal blue scrubs. Floating: Candidate may be expected to cover patients across the specified coverage areas.
