KEY RESPONSIBILITIES

• Provide comprehensive hospice nursing care to terminally ill patients in home and facility settings
• Conduct initial and ongoing assessments; manage pain and symptom control
• Educate patients and family caregivers on disease progression and end-of-life care
• Coordinate with hospice interdisciplinary team including social work, chaplaincy, and aide services
• Document clinical assessments, medications, and visit notes in EMR

REQUIREMENTS

Must-Have

• Active Texas RN licence or compact licence eligibility
• BLS certification current
• Minimum 1 year hospice, home health, or palliative care RN experience
• Valid driver's licence and reliable transportation

Preferred / Nice-to-Have

• CHPN certification
• Case management experience
