Responsibilities Provide direct patient care and implement RN care plans. Document visits accurately and timely. Educate patients and caregivers. Communicate changes in condition to RN. Follow physician orders and company policies. Qualifications Active NM LPN license. Current CPR certification. Valid driver’s license and reliable transportation.
