Community Action Partnerships of Western Nebraska (CAPWN) is a non-profit community-based health and human services organization that serves low-income, disadvantaged and those unable to meet their needs through other sources. The agency was established in 1965 and provides services to over 9,000 individuals, children and families in three main areas:

  • Community Health Services : WIC, Commodity Supplemental Food, Foster Grandparents, Head Start, and Child Development Center.
  • Clinical Health Services : Primary Care, Reproductive Health, Diabetes Management, Immunizations, Migrant Health, Ryan White HIV; Behavioral Health including Counseling Services, Medication Management, Medication Assisted Treatment and Substance Use Counseling and Intensive Outpatient Services and Dental Services.
  • Supportive Health Services : Family Stabilization, a Teen Outreach Program, SOAR, Youth Leadership Program, Youth Shelter and Housing Assistance Programs.

Job Title : Family Stabilization Case Manager

Reports to : Family Stabilization Manager

Areas of Responsibility: Implementation of the Family Stabilization (emergency assistance) and case management programs; home visits when needed; assist clients with the development of goal setting and action plans; advocate for clients; coordination of CAPWN services and other area resources; assessment of clients for eligibility of services for all programs, occasional assist with Special Distribution projects.

Major Duties:

  1. Complete initial client interview, required paperwork, and intake in agency data tracking system for emergency assistance.
  2. Assess, and assist families in the development of a self-sufficiency plan and track client progress in agency database.
  3. Work in conjunction with Family Stabilization Committee in determining client eligibility for assistance requests.
  4. Maintain confidentiality of all client records, information, and communications in accordance with CAPWN policy and HIPAA privacy and security requirements, while providing a supportive, trauma-informed environment for every client interaction.
  5. Adhere to all funding source guidelines.
  6. Make referrals to outside resources as needed.
  7. Provide case management support and assist families in meeting their goals.
  8. Coordinate with CAPWN programs and area agencies to recruit and coordinate services for client families.
  9. Provide training sessions for clients to develop self-sufficiency and life skills.
  10. Provide outreach services.
  11. Complete required reports for program and agency paperwork.
  12. Attend meetings and trainings as required.

  14. Perform other duties as assigned.
Working Conditions :
  • Office work, training, meetings, community interaction, occasional warehouse work and outdoor activities.
  • Works with clients with complex socio-economic, physical and health needs.
  • Work may be stressful and requires ability to meet tight deadlines.
  • Limited out of town travel.
  • Experience using Microsoft Word and Excel.
  • Will be using a computer, scanner, faxing, copier/printer and phone.
Physical Requirements:
  • Ability to bend, stoop, sit, stand, squat, crouch and pull file drawers
  • Occasional lifting and carrying of up to 30 pounds

Minimum Qualifications:

Grade: 2
  1. 1 year experience in a human service program or agency working with clients facing insufficiencies.
  2. Computer experience required.
  3. Proficient in verbal communication and in maintaining written documentation.
  4. Bilingual preferred, incentive pay available.
  5. Age 21+ preferred
  6. Valid driver's license with good driving record required and insured vehicle available is preferred.
Grade: 3
  1. Meet the minimum qualifications above.
  2. AA in Human Service, Social Work, Psychology or related field OR 3 years' experience in a Human Service agency working with clients facing insufficiencies.
Grade: 4
  1. Meet the minimum qualifications above.
  2. Bachelor's Degree in Human Services, Social Work, Psychology or related field OR 6 years' experience in a Human Service agency working with working with clients facing insufficiencies
