Registered Nurse (RN) Case Manager | MCK Case Management

Location Avera McKennan Hospital, United States of America – Day Shift Position Highlights Pay Range: $34.50 – $51.50 per hour (actual rate dependent on experience) May be eligible for a sign‑on bonus up to $15,000 Position Overview Facilitates the plan for an orderly transition of patients from the Med/Surg and medical ICU acute area settings to the next appropriate level of care. Works closely with physicians and payors to ensure proper use of fiscal resources and continuity of care. Responsibilities Provide courteous customer service to patients, families, and physicians Maintain collaborative relationships with providers, staff, and community agencies Deliver medical updates during hospitalization and at discharge Monitor documentation to ensure evidence‑based care for all age groups Complete medical record documentation thoroughly and promptly Maintain knowledge of coverage by managed care, private insurance, and government payers Coordinate all discharges and transfers (to acute to acute, tertiary centers, skilled nursing, long‑term care, assisted living, inpatient rehabilitation, long‑term acute care, group homes, hospice, prisons, etc.) Assist in transportation and service arrangements (home health, outpatient dialysis, infusions, durable medical equipment, oxygen, wound care, therapy services, NICU care programs, etc.) Participate in daily and weekly multidisciplinary rounds and patient care conferences Communicate with Patient Advocate Services for financial resources Coordinate with logistics for potential transfer back candidates Complete state‑required paperwork for skilled nursing or long‑term care facilities Work with state officials on difficult‑to‑place patients Assist with Medicare discharge messages and appeal process paperwork Serve as a resource for SOPs and standards of care to meet regulatory and participation standards Collect and maintain records for Medical Support Services, Joint Commission, OSHA, or legal requirements Essential Qualifications Registered Nurse (RN) with active license in the state of practice Ability to work the specified hours and perform all essential functions, including adequate vision, hearing, and effective communication Preferred Education / Certification / Experience Case Management Nurse – Board Certified (CMGT‑BC) with ANCC credential 4–6 years of hospital experience and/or training, or equivalent education and experience Expectations and Standards Commit to daily application of Avera’s mission, vision, core values, and social principles Promote compassion, hospitality, stewardship, and CARE (Communication, Attitude, Responsiveness, Engagement) Maintain confidentiality and work effectively in a team environment Comply with safety principles, laws, regulations, and standards (CMS, Joint Commission, DHHS, OSHA, etc.) Benefits PTO available day 1 for eligible hires Up to 5% employer matching contribution for retirement Career development guided by hands‑on training and mentorship Equal Opportunity Employer Avera is an Equal Opportunity Employer.
