Licensed Practical Nurse

Life Skills and Transition Center (LSTC) is looking for a Flex Overnight Nurse in Grafton, North Dakota. Position Summary Life Skills and Transition Center (LSTC) is looking for a Flex Overnight Nurse to provide compassionate, person-centered care to individuals with intellectual and developmental disabilities. The overnight nurse will support the interdisciplinary team and deliver high-quality nursing care, including medication administration, documentation, assessments, education, and emergency support. Responsibilities Administer medications and treatments accurately according to physician orders and individualized care plans. Maintain timely, thorough, and professional documentation in compliance with policies and regulations. Prepare medical information packets for transfers to hospitals or other care settings to ensure continuity of care. Implement physician orders promptly to provide the highest standard of medical care. Complete quarterly nursing assessments to evaluate health status, identify new or changing medical conditions, and monitor chronic health concerns. Provide education, guidance, and ongoing support to individuals, direct support professionals, medical technicians, and interdisciplinary team members. Serve as a trusted clinical resource by answering questions and collaborating with healthcare providers to address routine and urgent medical concerns. Assist with coordinating, conducting, and documenting emergency assessments and transfers as needed. Qualifications Licensed Practical Nurse (LPN) by the ND Board of Nursing or another state, with eligibility for licensure in ND. Licensed Registered Nurse (RN) by the ND Board of Nursing or another state, with eligibility for licensure in ND. Strong clinical judgment, organizational skills, and attention to detail. Excellent communication skills to interact effectively with individuals, families, physicians, support staff, and team members. Ability to remain calm under pressure and manage multiple priorities in a dynamic healthcare environment. Commitment to continuous learning, teamwork, and delivering exceptional care.
