This will be a full time or part time day shift- flexible hours/ schedule

Full time needs are all over the territory

Part time needs focus mainly on Centerville, Kettering, and Beavercreek area.

Pay Range: 42.50-48.80 (Base) pay per visit point system/ increases based on visit type

Position Summary:

Responsible for evaluation, plan of care, treatment, re-evaluation, discharge, documentation, and modification of all client programs. Administers physical therapy in compliance with Federal, State, local, and Bethany Village Home Health Care standards, policies, and guidelines.

Job duties:

• Administers physical therapy in compliance with Federal, State, local and Bethany Village Home Health Care standards, policies, and guidelines ensuring quality care and service to clients.
• Interacts with health professionals, clients, families, and support groups to ensure maximum rehabilitation, while serving as a client advocate at all times.
• Evaluates clients by administering muscle, neurological, gait, and functional tests.
• Establishes treatment goals based on the evaluation of the client in conjunction with the clients needs.
• Develops treatment programs and provides ongoing assessments and modifications as changes in the clients condition occur.
• Prepares, transmits, and maintains timely and accurate records to include the initial evaluation, progress notes, discharge summary, and attendance records in accordance with agency policy and that support charges for services provided.
• Promptly reports incidents, accidents, changes of condition, abuse, concerns to the assigned staff nurse or the Director of Bethany Village Home Health Care.
• Complies with OSHA standards and ensures the safety of the client in their care at all times.

Qualifications:

• Current Ohio Licensed Physical Therapist
• CPR certification.
• Valid drivers license and a safe driving record.
