Job Description Summary:

The purpose of the Wound & Ostomy Care Specialist is to coordinate an interdisciplinary team approach in the care of patients at MarinHealth Medical Center (MHMC) with altered skin integrity and alteration in elimination. The specialist will provide assessment, development, and implementation of the plan of care, evaluating the effectiveness of that care, directing emergency interventions as needed, and collaborating with physicians, team members, and others to optimally manage patient care in the interest of the patient, acting as a patient advocate when necessary.

Job Specifications:

Education:

• Bachelor’s Degree in Nursing required, and Master’s Degree preferred.

Experience:

• Minimum of 1-year acute care or home health experience

• Familiar with California regulatory bodies including: Joint Commission, Title XXII, D.H.S., and HIPAA

• Must be knowledgeable and competent in nursing procedures and technical skilled interventions

• Demonstrated leadership in clinical practice, education, and applied research as evidenced by meeting the specified criteria

License and Certifications:

• Registered Nurse Required at hire

• Basic Life Support Required at hire

• Integrative Agitation Management (IAM) Required within 30 days of hire

Prerequisite Skills:

• Must be a self-starter with a high degree of motivation, flexibility, energy, and creativity

• Ability to form harmonious working relationships with internal and external customers

• Demonstrated leadership ability to manage resources for patient care in a fiscally responsible manner

• Must have excellent collaborative and problem solving skills with managing the interdisciplinary wound care team, customers, nurses, physicians, and managers

• Possess the ability to exercise independent and sound judgment for assessing, planning, implementing, and evaluating patient care

• Excellent organizational skills and the ability to gather, interpret, and document data appropriate to the wound program and facilitate performance improvement activities

• Access to reliable transportation

• Ability to read, speak, write, understand, and follow the English language in both verbal and written format

Duties And Responsibilities:

Essential (Not Modifiable)

• Will oversee professional “best practices” of nursing and the interdisciplinary team providing wound, skin, and ostomy care to patients. Will also assist the organization in providing orientation, on-going education to nursing staff, and clinical staff directly providing care for wound, skin, and ostomy patients

• Consult with and utilize appropriate resources to continually improve practice, care, and services for patients and staff

• The Wound & Ostomy Care Specialist will be responsible for coordinating, directing, documenting, and completing a prevalence study.

• Assesses for and identifies patients at risk for developing pressure ulcers in coordination with the bedside care team

• Collaborates with all hospital staff and physicians to minimize the risk of skin breakdown

• Actively participates in hospital staff and physician education in methods of preventing the development of pressure ulcers to the in-patient population

• Participates in direct patient care for the purpose of implementing, monitoring, and follow-through of all wound & skin care treatment plans

• Provides consultation to Physicians and other members of the interdisciplinary care team in providing state-of-the-art and clinically efficient delivery of care to hospitalized patients

• Plans, coordinates, and implements teaching and management of patients with new or existing fecal or urinary diversion (ostomy). This role includes determination of supplies needed to meet inpatient needs and needs for patient discharge

• Provides recommendations, implementation, and utilization guidelines for the use of specialty beds

• Collaborates and consults with physicians and surgeons in determining the most appropriate and cost effective wound care and/or ostomy treatment plan

• Collaborates with the new products committee to oversee the provision and utilization of efficient and cost effective supplies for the wound and/or ostomy patient

• Assesses education needs of the nursing staff regarding wound care and ostomies. Assists with and provides education for the nursing staff regarding wound care and/or ostomies. Also provides programs and in-services as needed to maintain or enhance staff competency and improved patient care outcomes

• Identifies, monitors, and documents quality indicators, and provides written quality reports to the quality team upon request.

• Has the ability to function autonomously in the coordination, development, and implementation of the wound/skin/ostomy program at MarinHealth Medical Center
