Peer Health Navigator: Post-Crisis Community Reentry

Community Access

Community Access is looking for a Peer Health Navigator in New York City to connect Intensive Crisis Residence guests with community resources. This role is crucial for effective discharge planning to ensure stability after a crisis. Candidates should preferably hold a Bachelor’s degree and a New York State Peer Certification. Strong communication, analytical skills, and proficiency in computer programs are essential.
