Job Description:

The RN Case Manager role involves coordinating patient care, managing discharge planning, and working collaboratively within a medical facility to ensure quality and efficiency in healthcare provision. This position requires Meditech and Midas experience.

Responsibilities:

Case management assessment
Discharge planning

Requirements:

Minimum 1-year current acute Care Case Manager RN experience
BLS Certification
Active KS licensure
Preferred Midas and HCA case management experience
Prior HCA Hospital experience
