Description: The RN implements professional nursing practice centered on the patient as a whole person. The focus is on creating a caring environment centered on healing and wellness. Nursing practices are based on evidence and current research as well as hospital and department policies and procedures, regulatory standards, and professional guidelines. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines. Responsibilities include assuring direct patient care activities on the unit through provision of nursing rounds, assessments, and treatments. Facilitating admission, transfer, and discharge activities is essential. Education for patients and families is provided, along with care coordination and individualized treatment planning as applicable. The RN performs and documents a comprehensive assessment in accordance with scope of practice and policies. Cultural, age, and developmental considerations are accounted for in assessments. Identifying patient risk factors and obtaining health information from family or others is crucial. Objective and subjective data is collected and initial admission assessments verified. A problem list is developed, prioritizing patient needs based on input from the inter-professional team. Designing actions and interventions relevant to a patient’s culture and age is part of the role. Desired outcomes are determined in coordination with patients and families according to the medical plan of care. Implementation of patient care therapies and procedures is fully carried out with empowerment and clinical autonomy. Procedures, treatments, and processes are explained to patients and families. Action is initiated to alleviate patient stress and discomfort, with changes in patient condition recognized and communicated. Ongoing assessments are conducted and documented along with responses to procedures and therapies. Delegation of tasks is done appropriately, overseeing team members in patient care activities. The accuracy and completeness of care provided by other team members is monitored. Educational needs of patients related to prioritized problems are identified and addressed. Facilitating the patient discharge process is essential, including executing discharge orders in a timely manner. Input into patient/nurse assignments considers available staffing, unit activity, and patient acuity. Active participation in unit-based activities supports organizational values and priorities. Promoting a positive group dynamic and healing environment through actions and words is key. Nursing excellence is assured in accordance with accepted standards, evidence-based practice, and a caring philosophy. Participation in quality improvement activities and core measures is important, focusing on patient satisfaction and safety. A holistic perspective regarding patient care is utilized.
