Responsibilities
- Conduct initial evaluations and ongoing reassessments for residents requiring physical therapy in a SNF setting.
- Develop and implement individualized treatment plans to improve mobility, strength, balance, gait, and functional independence.
- Deliver therapeutic interventions (therex, manual therapy within licensure scope) and monitor patient progress toward goals.
- Document evaluations, goals, treatment plans, progress notes, and discharge summaries in electronic medical records; ensure accuracy and regulatory compliance.
- Collaborate with an interdisciplinary team (physicians, nurses, therapists, case managers) to support consistent care planning and smooth transitions of care.
- Educate residents and families on home exercise programs, safety considerations, and expectations for recovery and discharge.

Requirements
- Current Pennsylvania physical therapy license or eligibility for licensure
- Graduation from an accredited physical therapy program (DPT preferred)
- Experience in geriatric or long-term care rehabilitation is preferred
- Strong communication and documentation skills; ability to work within an interdisciplinary team
- Ability to work in a long-term care setting with a focus on functional improvement and safety
- Commitment to professional standards and ongoing learning
