About the Position

Job Responsibilities

• Perform comprehensive physical therapy evaluations and treatments for patients across a wide range of ages, diagnoses, and functional limitations.
• Assess patients, establish therapy diagnoses, develop individualized treatment plans, set goals, and implement appropriate PT interventions.
• Monitor patient responses and modify treatment plans as needed to optimize functional outcomes.
• Review clinical records, medical histories, and relevant patient information before treatment.
• Provide care to both inpatient and outpatient populations in various assigned settings.
• Collaborate with physicians, nurses, therapists, and other healthcare professionals regarding patient status, treatment plans, and discharge needs.
• Participate in care conferences, rounds, team meetings, and communication with referring providers.
• Oversee and direct the work of Physical Therapist Assistants, students, interns, and designated support staff.
• Develop and provide patient and family education related to diagnosis, functional limitations, therapeutic exercises, mobility, and self-care.
• Create clear and individualized home exercise programs and confirm patient/caregiver understanding through feedback and return demonstrations.
• Assist patients and families in developing skills that promote independence and maximize rehabilitative potential.
• Assist with the selection, acquisition, fitting, and delivery of durable medical equipment and assistive devices based on patient needs.
• Document evaluations, treatments, diagnoses, treatment plans, recommendations, and prognoses according to facility policies and regulatory requirements.
• Maintain accurate and timely documentation and provide required reports, statistics, and performance metrics.
• Maintain clinical equipment, supplies, and treatment areas in a clean and organized condition.
• Stay current with applicable billing procedures, including Medicare, Medicaid, and third-party payer requirements.
• Provide PT-related education, presentations, consultations, and in-service training to staff and interdisciplinary healthcare teams.
• Participate in committees, management activities, quality improvement initiatives, and rehabilitation department activities as assigned.
• Maintain compliance with American Physical Therapy Association (APTA) practice standards and applicable regulatory requirements.
• Maintain patient confidentiality and follow all quality assurance and safety standards.
• Perform other duties as assigned.

Requirements

Required Qualifications

• Master's or Doctorate degree in Physical Therapy from an accredited program, OR graduate of an APTA-approved Physical Therapy program at the bachelor's or master's level with qualifying work experience and continuing education.
• Current and unrestricted Physical Therapist (PT) license.
• Current Basic Life Support (BLS) certification.
• Valid driver's license.
• Previous Physical Therapy experience required.
