Clinical Care Coordination Relationship Building: Develop and maintain supportive, ongoing relationships with young clients and their caregivers using strengths-based interventions. 2. Systems Collaboration & Administration Teamwork: Collaborate with multi-disciplinary teams and external partners, including physicians, schools, therapists, and community providers. Waiver Management: Update Plans of Care for Severe Emotional Disturbance (SED) waiver patients (30 days post-start, and every 90 days thereafter) and coordinate with Utilization Review for service authorizations. Timely Documentation: Maintain exceptionally accurate records. Our team standards include completing 90% of progress notes within 72 hours, completing waiver documents/budgets within two weeks of a case conference, and submitting service verifications within 72 hours. 3. Professional Growth & Community Connection On-the-Job Mobility: Travel locally to meet clients where they are, ensuring reliable transportation and community-based support.
