Physical Therapist – Home Health

Schedule / Job Type

Contract | Full-Time | Days (5x8 Hours, 8:00–16:30) | Onsite / Field-Based

Key Responsibilities

• Provide skilled physical therapy services to home health patients in Seaside, OR and surrounding areas
• Conduct patient visits in a home health setting, seeing up to 6 patients per day with a standard productivity expectation of 30 points per day
• Complete Oasis assessments as required for home health documentation
• Chart using EPIC Remote Client for Home Health (laptop) and Rover mobile application
• Manage a daily caseload with an occasional maximum travel radius of approximately 60 miles, with total daily drive time up to 60–90 minutes

Required Qualifications

• Minimum of 1 year of skilled home health physical therapy experience
• Active or pending Physical Therapist license in the state of Oregon
• Basic Cardiac Life Support (BCLS) certification required
• Demonstrated proficiency with Oasis documentation
• Reliable personal transportation required
• COVID-19 primary vaccination series plus one booster required

Preferred Experience / Skills

• Familiarity with EPIC Remote Client and Rover application for home health charting
• Experience meeting productivity point expectations in a home health environment
• Comfort working independently in a field-based, community care setting
