Key Responsibilities

• Conduct physical therapy evaluations and provide treatment for home health patients across a multi-county rural service area
• Complete OASIS Assessments and manage Start of Care cases in accordance with home health standards
• Perform approximately 4–6 patient visits per day based on a point-equivalent productivity system
• Utilize a provided tablet for real-time documentation of time and patient visits
• Treat common patient populations including elderly individuals with fall-related injuries and fractures

Required Qualifications

• Active Physical Therapist (PT) license in the state of Indiana
• Experience completing OASIS Assessments and Start of Care evaluations
• Ability to travel throughout a rural, multi-county service territory
• Comfortable using a tablet-based documentation system in the field

Preferred Experience / Skills

• Prior home health physical therapy experience
• Experience treating geriatric patients, including fall prevention and post-fracture rehabilitation
