Position Overview The Licensed Practical Nurse (LPN/LVN) is responsible for providing and documenting skilled nursing care, under the supervision of a Registered Nurse, in accordance with the care plan and physicians’ orders for each individual patient while adhering to confidentiality standards and professional boundaries at all times. Essential Job Functions Deliver and coordinate quality patient care in compliance with physician orders. Continuously observe and assess patient condition and care needs; report changes to supervisor and/or physician as appropriate. Document all activities, assessments, nursing actions, responses, and coordination of care timely when care is delivered. Participate, implement, and update the nursing care plan. Takes appropriate nursing action based on assessment and achieves expected outcomes. Recognize changes in patient needs and responses requiring intervention; implement care to prevent or reduce risk. Accept responsibility for personal and professional accountability by complying with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act. Apply infection control measures protecting staff and patient per OSHA standards. Educate patient and family regarding disease process, self‑care techniques, and prevention strategies. Maintain knowledge of nursing competencies through educational programs, continuous education units, internal learning management, and skill evaluations. Qualifications Active, unencumbered license (LPN/LVN) in the state of practice; compact licenses transferred within 90 days. Current CPR certification (hands‑on component). Aveanna can assist in obtaining if necessary. TB skin test current within last 12 months. Six months prior hands‑on nursing experience preferred but not required. Reliable transportation. Requirements Graduate of an accredited school of nursing. Current, unrestricted state license as an LPN/LVN. Current CPR certification. Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care policies and procedures. Additional State‑Specific Requirements South Carolina – 1 year of pediatrics experience. California – 1 year of experience under current nursing license. Louisiana – 1 year of experience as a licensed nurse. Continuing education as required by state. Preferences Six months of recent experience as an LPN/LVN in a clinical care setting. Home health experience. Other Skills/Abilities Attention to detail. Time management. Effective problem‑solving and conflict resolution. Good organization and communication skills. Physical Requirements Speak, write, read, and understand English. Travel capability. Lift 50 pounds. Reposition patients and move equipment without assistance. Prolonged walking, standing, bending, kneeling, reaching, twisting. Sit and climb stairs. Visual and hearing acuity. Strong sense of smell and touch. Respond physically and mentally to emergency situations in the home or during transport. Environment Function in a wide variety of environments involving exposure to allergens and other conditions.
