Case Manager
Shreveport Rehabilitation Hospital
Shreveport Rehabilitation Hospital is seeking a Case Manager who collaborates with physicians to manage individual patient programs and ensure progression through the care continuum. You will monitor clinical and financial coordination of treatment plans to achieve quality outcomes while aligning with CMS guidelines. Responsibilities include coordinating length of stay, supporting patients with spinal cord injuries, brain injuries, strokes, amputations and other rehab needs, and ensuring #J-18808-Ljbffr
