Description

• Conduct evaluations for patients in inpatient or outpatient environments and create tailored treatment plans
• Implement physical therapy interventions that enhance mobility, strength, balance, and overall function

• Guide patients and their families in understanding therapeutic objectives, discharge planning, and home exercises
• Maintain thorough documentation of evaluations, treatments, and patient progress as per facility and regulatory requirements
• Engage in care team discussions and interdisciplinary meetings to ensure cohesive care delivery

Education

• Doctorate in Physical Therapy (DPT) or equivalent credential from an accredited institution

Certifications

• Active state licensure as a Physical Therapist
• CPR certification preferred
