Position Summary Vitae Healthis is seeking a dedicated Nurse Practitioner to provide high‑quality, patient‑centered medical care to residents in a skilled nursing facility. The NP will assess, diagnose, and manage acute and chronic conditions while collaborating with an interdisciplinary care team to improve patient outcomes and quality of life.
