Job Summary
We are seeking a dedicated Respiratory Therapist - COPD Transitional Navigator to provide specialized respiratory care, patient education, and care coordination for COPD patients. The role involves supporting patients throughout their care journey, promoting disease management, reducing readmissions, and ensuring safe discharge planning.
Required Qualifications
Graduate of an AMA-approved Respiratory Therapy program.
Current and valid New York State Respiratory Care License.
Registered Respiratory Therapist (RRT) credential or eligibility to obtain within 6 months of hire.
Current CPR/BLS certification.
ACLS certification.
Strong knowledge of COPD, pulmonary diseases, and respiratory assessment
