Job Title MO-SSM Health – Saint Louis University Hospital – PRN (Per Diem) Worker Job Summary Provides social work assistance to patients and families related to social determinants of health. Responsibilities Interview patients, assess priorities, and document case activity. Consult with multidisciplinary team and link patients to community resources to support treatment. Assist patients/families in obtaining community assistance via referrals. Develop and maintain relationships with community health, welfare, and social agencies, seeking creative solutions to meet patient needs. Participate in team efforts to identify opportunities for process improvement. Maintain a constant state of alertness and work in a safe manner. Perform other duties as assigned. Requirements Education: Master’s degree in Social Work. Experience: No prior experience required. Professional license: None required. Physical Requirements Frequent reaching, sitting, standing, walking; use of vision, hearing, speech, keyboard; occasional bending, lifting (0‑25 lbs), driving; rare repetitive movements, squatting.
