Field Care Coordinator

Become a part of our caring community We are looking for motivated and dynamic case managers that reside in Greenacres, Lake Worth & West Palm Beach, FL. Current starting salary is $64‑68K/year based on qualifications and experience. Care Coach 1 Responsibilities Visit Medicaid members in homes, assisted living facilities, long‑term care facilities, and other care settings – 75‑90% local travel. Assess and evaluate member needs and requirements to establish a member‑specific care plan. Ensure members receive services in the least restrictive setting to achieve and/or maintain optimal well‑being. Plan and implement interventions to meet those needs. Coordinate services, monitor, and evaluate the case management plan against the member's personal goals. Guide members/families toward appropriate resources for their care. Facilitate interactions with other payer sources, providers, interdisciplinary teams, and others involved in the member’s care as required by the comprehensive contract. Required Qualifications Bachelor's degree with at least 2 years of relevant experience (Health Services, Social Work, Psychology). Registered Nurse (RN), licensed in Florida, with at least 2 years of relevant experience. Licensed Practical Nurse (LPN), licensed in Florida, with at least 4 years of relevant experience. Case Worker with a minimum of 6 years of Health Services/Case Management experience. Prior experience with Medicare & Medicaid recipients. Intermediate to advanced computer skills and experience with Microsoft Word, Excel, and Outlook. AHCA fingerprint required. TB screening required – role is patient‑facing and part of Humana’s TB screening program. Valid Florida driver’s license and personal vehicle liability insurance – part of Humana’s Driver Safety program. Availability to work Monday–Friday 8:30 am–5:00 pm. Preferred Qualifications Bi‑lingual English/Spanish with a language proficiency exam. Community Health Worker experience. Prior nursing home diversion or long‑term care case management experience. Experience with electronic case note documentation across multiple computer applications/systems. Experience working with a geriatric population. Experience with health promotion, coaching, and wellness. Knowledge of community health and social service agencies and additional community resources. 701B Certification.
