Registered Nurse – Home Health

Schedule: 8 hours per shift

Assignment Details:

• Contract Length: 11 weeks
• Guaranteed Hours: 40 per week

Requirements:

• Current state license as a Registered Nurse
• Homecare Homebase Software Experience
• OASIS Experience
• Proof of current CPR certification
• Current driver's license
• Reliable transportation and insurance
• 1-2 years experience as a Registered Nurse in a clinical care setting
• 2+ years Home health experience
• CHPN certification (Preferred)

Roles & Responsibilities:

• Assess patients in home health and hospice settings to identify their needs.
• Develop, document, and evaluate the Plan of Care to ensure consistent quality of care.
• Educate patients and families on care procedures, medication administration, and home safety.
• Supervise and provide clinical direction to support staff.
• Monitor pain management techniques and educate patients and families accordingly.
• Maintain up-to-date patient records and ensure compliance with third-party payer requirements.

Compliance / Notes:

• Perform on-call responsibilities as assigned.
• Attend necessary interdisciplinary team meetings.
