RN Care Manager: Discharge & Care Coordination Leader

TEEMA Solutions Group seeks a Clinical Coordinator / RN Care Manager in White Plains, NY, responsible for comprehensive patient assessments and multidisciplinary care coordination. The role requires a BSN and a Registered Nurse (RN) NYS license, with a preference for candidates having clinical experience in emergency, critical care, or med/surg settings. The Clinical Coordinator will help ensure effective discharge planning and collaborate with healthcare professionals to prevent readmissions.
