Your Role & Responsibilities: As a home care LNA, you will provide peaceful, dedicated personal care and clinical support to our clients in the comfort of their homes. Companionship & Connection: Engage clients in meaningful conversation, puzzles, walks, and activities to keep them socially active and mentally sharp. Dignified Personal Care: Provide hands-on support with Activities of Daily Living (ADLs) including bathing, grooming, dressing, skin care, and personal hygiene. Safe Mobility & Transfers: Assist clients with ambulation, transfers, and positioning using proper body mechanics and assistive devices. Meal Prep: Assist with meal planning and nutritional cooking according to dietary restrictions; monitor and encourage hydration. Medication Reminders: Provide timely reminders for clients to take prescribed medications and support routine adherence. Light Housekeeping: Maintain a safe, clean, and hygienic environment by handling laundry and light cleaning. Companionship & Transport: Engage clients in meaningful social interaction and accompany or drive them to local medical appointments and essential outings. Clinical Observation: Closely observe, document, and report any changes in the client’s physical or mental status to the nursing supervisor. Job Requirements (Qualifications): Active New Hampshire LNA (Licensed Nursing Assistant) certificate in good standing (Required). 1+ years of experience as an LNA in home health, a facility, or a hospital setting preferred. Current certification in CPR and First Aid (Preferred). Strong communication skills and a reliable work ethic.
