Work Environment\n

This role is primarily remote, allowing social workers to collaborate with multidisciplinary teams effectively while providing services from the comfort of their homes. However, occasional visits to our hospice center in Oyster Bay may be necessary for team meetings or patient engagement activities. Patient Population / Client Focus\n

In this role, you will primarily work with adults and their families facing terminal illnesses. Through a compassionate approach, you will provide support that addresses psychological, emotional, and relational dynamics, ensuring that patients receive holistic care that values their dignity and wishes. You will also assist families in coping with grief and loss, guiding them toward resources that promote healing and resilience.

Qualifications\n

• Bachelor's Degree in Social Work or a related field is required.

Compensation and Benefits\n
