Hospice Registered Nurse (RN)

Hospice Registered NurseSupplemental Health Care is hiring compassionate and experienced Hospice RNs to provide patient-centered care for individuals and families in Moab, Utah, and surrounding areas.Qualifications:Active, unencumbered RN license in the state of assignmentCurrent BLS certification (American Heart Association)CHPN certification preferred, not required1+ year of recent experience (within last 3 years) in hospice, home health, palliative care, or acute careStrong knowledge of symptom management, pain control, and hospice documentation (CMS CoPs, Medicare Hospice Benefit)Clear background and driving record; current TB test and immunizationsReliable transportation and valid auto insuranceFlexibility for travel, occasional evenings, weekends, and on-call shiftsHospice RN Contract Details:$1,746 - $1,931 per week*Nurse will have a designated territory of coverage4- 6 patient visits per day (45 - 90 minutes each, depending on need)Travel time included in schedulingAll nurses on contract with SHC are paid weeklyKey Responsibilities:Perform comprehensive assessments and develop individualized hospice care plansDeliver skilled nursing care, symptom management, and emotional supportEducate patients and families on hospice philosophy and comfort care practicesCollaborate with the interdisciplinary team to review and adjust care plansMaintain accurate documentation in compliance with hospice standardsWhether you want to travel or stay local, we help Hospice RNs find the perfect fit with leading healthcare organizations nationwide.
