• Full-time, permanent
• Outpatient Only
• Flexible schedule; Four 10 hour days (8 hours seeing patients/2 hours for charts) or five 8 hour days, Monday thru Friday
• Four year service commitment specifically tied to the sign on bonus and relocation.
• 72 patient per week via clinical encounters

Compensation Details
• Year 1 Base Salary = $300,000
• Year 2 & beyond - Annual performance inventives
• $50.20/wRVU in excess of 6,00 Annual Productive Bonus
• Locations qualify for state & federal student loan repayment (Primary Care Loan Forgiveness - )
• Signing Bonus: $60,000
• 15K relocation
• 5K CME
• 240 hours PTO
