Clinical Pharmacist - Pediatrics

Job Description Inova Fairfax Hospital Pharmacy is seeking a dedicated Clinical Pharmacist – Pediatrics/PICU to join the team. This role will be full‑time, with shift variations. Clinical Pharmacist Job Responsibilities Interpret, prepare, and verify medication orders as appropriate. Provide clinical consultation and cost‑effective alternatives. Issue controlled substances in compliance with DEA and VA Board of Pharmacy requirements. Participate in medication formulary review and approval processes. Monitor drug therapies for interactions, allergies and appropriateness. Report adverse drug reactions promptly and accurately. Minimize non‑formulary procurement and support substitutions. Supervise pharmacy support staff. Participate in continuous quality improvement. May perform additional duties as assigned, including, but not limited to: Participate in decentralized rounding with clinical teams. Respond to emergent bedside alerts and codes (adult and pediatric as designated). Precept learners as appropriate. Minimum Qualifications Education: Doctorate of Pharmacy (PharmD) or Bachelor’s degree in Pharmacy. Licensure: Licensed Pharmacist in the state of VA. Certification: Basic Life Support (BLS), Advanced Cardiovascular Life Support (ACLS) for Critical Care and Emergency Medicine, and Pediatric Advanced Life Support (PALS) within 90 days of hire. Preferred Qualifications Experience: One year in Pediatrics and/or PICU as a Clinical Pharmacist. EPIC trained. PGY‑1 residency in Pediatrics/PICU.
