Key Responsibilities\n- \n
- Conduct thorough bio-psychosocial evaluations to assess patient needs and identify barriers to treatment, playing a crucial role in optimizing discharge plans. \n
- Collaborate with medical and support staff to devise and implement tailored treatment plans that incorporate evidence-based practices, ensuring each patient receives holistic care. \n
- Document social work activities accurately in patients' medical records, adhering to departmental standards and maintaining compliance with regulatory requirements. \n
- Facilitate both individual and group therapy sessions, utilizing diverse treatment modalities to address issues such as grief, anxiety, and adjustment disorders related to terminal illness. \n
- Provide education and resources to patients and families, guiding them through the healthcare process and helping them understand necessary medical and social work recommendations. \n
- Advocate for patients’ rights and needs within the community, connecting them with essential resources and social support systems. \n
- Remain current on community resources and actively integrate these into care plans to enhance patient experiences. \n
- Commit to continuous professional development through training and educational activities that enhance clinical skills and knowledge. \n
The primary focus will be on individuals in hospice care, along with their families, who require emotional support during a critical phase of life. Our patient population often faces complex psychosocial challenges, including end-of-life issues, grief, and mental health difficulties associated with chronic illness.
Qualifications\n- \n
- A Master’s Degree in Social Work from an accredited institution and current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Colorado. \n
- A minimum of one year of clinical experience working in a healthcare or mental health setting, with a preference for experience in hospice or end-of-life care. \n
- Demonstrated ability to apply clinical skills while considering developmental, cultural, and systemic factors impacting patient care. \n
- Proficiency in utilizing treatment planning techniques and assessing mental health needs in diverse populations. \n

- Experience with interdisciplinary team collaboration in providing mental health services within a hospice or palliative care setting. \n
- Familiarity with community resources and patient advocacy initiatives relevant to hospice care. \n
- Previous experience in training or mentoring junior social work staff or interns. \n
We offer a competitive salary range of USD 85,000 - 100,000 annually, commensurate with experience and qualifications. In addition, we provide a meaningful sign-on bonus to welcome new team members.
