Shift: Monday - Friday | 8:00 AM - 5:00 PM

As a Physical Therapy Assistant - Home Health, you’ll play a hands‑on role in helping patients regain strength, mobility, and confidence. Working under the guidance of a licensed Physical Therapist, you’ll deliver personalized care to a diverse community‑based caseload. Responsibilities

• Provide skilled physical therapy services in patients’ homes under the direction of a Physical Therapist and physician plan of care.

• Deliver hands‑on interventions to reduce pain, restore mobility, and improve functional independence through therapeutic exercise, gait training, transfers, prosthetic/orthotic support, and use of physical agents.

• Educate patients, families, and caregivers on safe mobility techniques and equipment use whilemonitoringpatient response and proactivelyidentifyingchanges in condition or risk for hospitalization.

• Completetimely,accurateclinical documentation in compliance with regulatory, payer, and accreditation standards.

• Participate in case conferences, record reviews, and quality improvement initiatives to support safe, effective, and coordinated care.

Qualifications

Education:

• Graduate of a Physical Therapist Assistant curriculum approved by the Commission on Accreditation in Physical Therapy Education of the American Physical Therapy Association; or, if educated outside the United States or trained in the United States military, graduated from an education program determined to be substantially equivalent to Physical Therapist Assistant entry‑level education in the United States by a credentials evaluation organization approved by the American Physical Therapy Association.

• Passed a national examination for Physical Therapist Assistants

Licensure/Certifications:

• Licensed, registered, or certified as a Physical Therapist Assistant by the state(s) providing services.

• Valid licensed driver with automobile insurance in accordance with state(s) and/or organizational requirements.

• Mandatory Reporter: Child & Dependent Adult Abuse

• CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification.

Minimum Experience:

• One year experience caring for similar patient population as to be assigned.

Other:

• Use of usual and customary equipment used to perform essential functions of the position.
