Physical Therapist - Outpatient

Job Description

The Physical Therapist (PT) will evaluate assigned patients, develop, implement, and modify treatment plans according to patients’ needs, goals, and physician direction. This role entails delegating and directing therapy services, ensuring the effective design of treatment programs to facilitate community reintegration. The PT will actively educate patients, families, and peers while adhering to the organization’s Mission Statement and Core Values.

Responsibilities

• Conduct comprehensive evaluations of assigned patients.
• Develop treatment plans and goals to ensure progression toward discharge.
• Document patient care clearly and accurately in compliance with guidelines.
• Provide educational support and supervision for students as needed.
• Create a professional development plan and pursue continuing education opportunities.
• Adhere to the State Practice Act and maintain professional licensure.
• Participate in performance improvement initiatives.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 6 months to 1 year of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state licensure for physical therapy.
• Previous experience with Select Medical is highly desired.

Schedule

This position requires a commitment of 40 hours per week with a flexible schedule.
