This position requires travel to several counties but offers a consistent schedule to fit your personal / family commitments (no overnight shifts or travel). This is a long-term employment opportunity with schedule flexibility and autonomy. More about the RoleThis role requires a mix of RNCM services and recertification visits - and travel - as needed to all clients across our South / Southeast Ohio region. Work consists of: case management, starts of care (SOC), admissions, recertifications, transitional care and resumptions of care (ROC), clinical reviews, and other assessments and services, as well as new hire orientations, in-service and supervisory reviews, and input to influence our training and education, care practices and outcomes, and the success of our clients, employees and business overall. *Services may also require occasional/rare weekly visit(s). At least 1 year of experience is required in: Case Management, Case Admissions and Discharges, wound care, PICC/PORT/IV Care, Transitional Care, lab draws, OASIS and Non-OASIS assessments, and excellent documentation and field staff leadership.At least 1 year of paid experience is required at/with a home health agency or in-home care provider (or equivalent); 2-5 years' experience is preferred.Must have experience with OASIS and NON-OASIS assessments.
