Key Responsibilities\n

  • Conduct comprehensive intake assessments to determine clients’ mental health needs, strengths, and areas for improvement.

  • Develop and implement individualized treatment plans tailored to the unique needs of each client, using evidence-based approaches.

  • Provide mental health therapy for individuals, couples, families, and children, addressing a range of emotional and psychological challenges.

  • Facilitate group therapy sessions and support groups, promoting community and shared experience among hospice patients.

  • Collaborate with the Utilization Review Department and Business Department to ensure timely authorization and payment for services rendered.
  • \n
  • Maintain meticulous and accurate documentation of patient interactions, progress notes, and treatment plans to comply with regulatory standards and enhance care continuity.

  • Engage with local resources to connect clients with necessary support services and offer interpretation and translation services as needed.

Patient Population / Client Focus\n

In this role, you will primarily work with hospice patients, their families, and caregivers, ensuring that they receive psychological support during emotionally challenging times. Your expertise will assist clients in coping with grief, terminal illness, and related mental health issues, helping them find peace and acceptance in their journey.

Qualifications\n

  • A Master’s degree in Social Work from an accredited institution.

  • Current, active license as a Clinical Social Worker (LCSW) in California is required.

  • Proven effectiveness in written and verbal communication skills, facilitating clear conversations with clients and interdisciplinary teams.

  • Demonstrated ability to establish and maintain professional boundaries while providing empathetic care.

  • Capacity to manage and complete documentation of services efficiently, ensuring compliance with clinical standards.

Compensation and Benefits\n

The salary for this position ranges from USD 120,000 to 130,000 per year , commensurate with experience and caseload.
