What You'll Do

Connect patients to healthcare and community resources
Help individuals overcome barriers to care
Support chronic disease management and healthy lifestyles
Provide health education and advocacy
Work alongside providers, nurses, and care coordinators
Participate in community outreach activities

Qualifications

High school diploma or GED
Strong communication and relationship-building skills
Knowledge of community resources preferred
Bilingual (English/Spanish) preferred
CHW certification or willingness to obtain certification preferred
