Practice ManagerOur Mission: To improve lives by providing convenient, comprehensive, high-quality healthcare to every person in every community that we serve.Our Vision: CCHN will maintain its position as the leading community health center in the state and become a model for all other healthcare organizations to follow.Major Function: To provide friendly, professional and high-quality nursing care to CompleteCare patients within a team-based, patient-centered medical home model of care.Essential Responsibilities:Provide nursing services to patients while ensuring a safe environment of careReduce risks of nosocomial infections in patients, employees and visitorsEmbody friendly and courteous customer service at all times with both patients and staffAct as an active participant in the healthcare team – working together with all levels of staff to ensure the best care possible for patientsWork with team to meet specific healthcare performance improvement goalsAdhere to all applicable HIPAA guidelines so as to ensure confidentiality of patient informationProblem-solve issues as they arise in a pro-active and positive mannerOrganize and prioritize tasks as assignedWork with supervisor to plan departmental activities, evaluate data and make suggestions for improvementMaintains clean and orderly health center and efficiently utilizes suppliesOther Job Duties:Identifies patientsPrepares patient recordPrepares patient for examAdministers prescribed medicationsAssists provider during examTakes and records accurate vital signsSelects and completes all required forms for medical visitsMaintain and update emergency boxes and oxygenReviews discharge instructions with patientsPerforms charge responsibilities as assignedCompletes logs and syringe countsCompletes medication logs and scanningOversees stocking of all medical suppliesPerforms and documents call-back/triage responsibilitiesMaintains immunization statisticsRenews prescriptions in accordance with approved refill guidelinesMonitors each chart in awaiting reply at nursing box and responds as necessaryPerforms nursing box transcriptions of orders correctly for each chart as assignedCompletes patient orders in the electronic medical recordEnsures completion of all necessary templates in the electronic medical recordQualifications:Graduate of an accredited nursing program1 – 2 year of experience preferredA current license from the State Board of NursingCPR certifiedExcellent communication and problem-solving skillsAbility to navigate electronic medical record systemsExcellent interpersonal skills and the ability to work effectively with all members of the healthcare team.Commitment to professional growth and development in a dynamic, forward-thinking organizationDemonstrated knowledge in role of every medical emergencyThe above job description is not to be construed as a complete listing of the assignments that may be given to any employee, nor are such assignments restricted to those precisely listed in the description.
